In our view, a proactive approach to complex care must be taken by care providers.

We believe that care should not only support the current needs of individuals but also work to prevent further issues from arising. This also includes anticipating future needs, for example through medical conditions progressing over time.

Unlike reactive care, proactive care removes all uncertainties for our clients and their families. Through this, we ensure that a high quality of care is maintained.

Adapting care plans

As well as a proactive approach to care, we are proactive with how we structure our care plans.

Each of our clients is assessed by a Registered Nurse, who meets with the client, their family and their commissioner. This meeting allows our clients and their loved ones to find out more about our services and advocate for the support that they need.

From here, a bespoke care plan proposal is drawn up that aims to provide the complex care needed for our clients to live a fulfilling life. This proposal can be adapted and discussed at any point.

We work alongside our clients, their families, and commissioners to ensure that our care plans are adjusted to fit their needs.

Through this proactive approach, we can ensure that our clients have all the tools they need, when they need them, including all equipment and supplies that their complex condition requires.
