TA was supported to move out of a residential service into his own home closer to his family. He settled in really well and built a good relationship with his team. TA began accessing the community daily and seeing his family every week. He has since moved from that property into a new property and independence is growing. TA was involved in the move and was empowered to say what items when into the van next and where things were going to be placed in his new home.
